Information on Telehealth:
In accordance with the updated regulations issued on April 22, 2020, by Early Intervention, we kindly request your cooperation in adhering to the following checklist before submitting your session notes:
E-mail Consent: Prior to engaging with parents via email, it is imperative that providers obtain signed consent. A sample consent statement is as follows: "This email/text message serves as my written consent on (name of the applicable form from the list below) in order to facilitate the Early Intervention process during the COVID-19 Declared State of Emergency (full name of parent/guardian providing consent) (date)."
Electronic Signatures: Please note that electronic signatures are strictly prohibited on service logs.
Inclusion of Teletherapy Statement: It is mandatory to include the statement "the service was delivered utilizing a teletherapy approach during the COVID-19 declared state of emergency" in each session note. We recommend including this statement in the "Additional Information" section.
Parent's Signature Line: Ensure that the parent's signature line on the session note specifies whether it was a "Telehealth session" or a "Virtual Session." It cannot be left blank.
Relationship to Child: Clearly indicate with whom the virtual session took place. This field should not be left blank.
Location: The session note must specify where the virtual session took place. If it occurred at home, indicate "Home/Virtual Telehealth." This field cannot be left blank.
Matching Date and Time: Please ensure that the date and time on both the NYC EI service log and the session notes match the actual, true time of the session. This alignment is crucial to prevent any time conflicts with other providers or agencies. Differing times may result in denial of billing, leading to payment delays. Our aim is to help you avoid such issues.
Cancellation/Missed Sessions/Gap Documentation: Even for sessions that are missed, please ensure that the cancellation, missed session, or gap documentation is completed, along with a clear explanation of the reasons for the session's absence.
Data Collection: Daily data or ABA program book entries must continue to be collected as per your routine practice.
